Person-centred support planning

Support planning that delivers outcomes tailored to individuals is a key part of the work of support staff. As services become stretched, staff need to be able to develop relationships quickly, using all the information that is available to work with clients in delivering person-centred support. Engaging with clients, record-keeping and delivering outcomes need to be carried out effectively and efficiently.
